Blood troponin I assay is used to diagnose or exclude acute myocardial infarction.

Troponin I is measured with 5th-generation reagents (high-sensitivity Troponin I, hs-cTnI) with much higher accuracy, sensitivity, and specificity than previous generations of reagents.

Troponin is a protein complex that regulates the contraction of striated muscle tissue. It consists of 3 subunits (C, T, and I) periodically located along the muscle fibrils. Troponin I is present in 3 isoforms: cardiac muscle, slow contraction skeletal muscle, and rapid contraction skeletal muscle. The cardiac isoform of troponin I (cTnI) is different from the other two isoforms, which allows the development of specific methods for its detection. This specialization improves the accuracy of diagnosis in patients with acute or chronic skeletal muscle injury and possible concomitant myocardial injury.

Troponin I is released into the bloodstream within hours of the onset of symptoms of myocardial infarction or ischemic injury. Troponin I can be detected from 3 to 6 hours after the onset of chest pain, with peak concentrations occurring at 12 to 16 hours and remaining elevated for 5 to 9 days.

  • Troponin I increase: Acute myocardial infarction, angina, coronary syndrome, myocarditis.
