Rose Bengal agglutination test is used as an initial screening test for patients with suspected brucellosis.

Various antibody classes are produced during a human infection by any pathogenic microorganism. Among these antibodies are agglutinins. When combined with the homologous antigen (agglutinogen) under controlled conditions, an agglutinin can cause agglutination.

Rose Bengal agglutination test uses Brucella microbial antigen for early detection of adhesins against species of Brucella (Brucella abortus, Brucella melitensis, and Brucella suis). This test is an initial screening test only for detecting agglutinins against Brucella. If the result is positive, it should be confirmed by other more specific serological tests for brucellosis.

Detectable agglutinins do not always indicate bacterial infection. In some cases, nonspecific agglutinins may react with Brucella antigens. For example, sera from certain classes of drug addicts have been reported to contain significant amounts of agglutinins. In some cases, cross-reactions may occur, such as tularemia. In addition, vaccinations can produce agglutinins capable of reacting with soluble antigens.
