Serum lipoprotein (a) measurement is used to assess the risk of developing cardiovascular disease in patients with a moderate or high risk of developing a cardiovascular disease based on conventional risk assessment indices.

Lipoprotein alpha or Lp(a) is a heterogeneous group of lipoproteins consisting of an ApoA molecule joined to an ApoB molecule. Elevated levels of Lp(a) are an independent risk factor for atherosclerosis and are particularly harmful to the endothelium. Serum Lp(a) concentration appears to be highly correlated with genetic factors whereas diet and drugs (statins) have no significant effect on lipoprotein (a) levels. Taking niacin, however, may lower Lp(a) levels.

Measurement of serum Lp(a) may contribute to a more in-depth assessment of the risk of developing an atherosclerotic disease in patients and other risk factors. Such patients are for example patients with early atherosclerosis, patients with a strong family history of premature coronary artery disease, patients with elevated LDL cholesterol and two or more risk factors, patients who have had coronary angioplasty and who may have an increased risk of restenosis, and patients who have undergone aortic bypass surgery and in whom increased lipoprotein alpha may be associated with graft stenosis.
