Detection of hemosiderin (hemosiderinuria) is used in the investigation of hemolysis, as occurs in incompatible blood transfusions, in severe acute hemolytic anemia, or in hemochromatosis.

When the plasma free-hemoglobin levels are from 50 to 200 mg / dL after hemolysis, the ability of haptoglobin to bind free hemoglobin reaches its limits, and free hemoglobin passes easily through the kidneys. Part of the hemoglobin is absorbed by the cells of the proximal renal tubules, where the hemoglobin iron is converted to hemosiderin. When these cells subsequently drop into the urine, hemosiderinuria emerges. If hemoglobin cannot be absorbed by the cells of the proximal renal tubules, hemoglobinuria results.

This test includes a microscopic examination of the urine precipitate for the characteristic large blue hemosiderin granules when potassium ferrocyanide is added to the sample.

Hemosiderin (iron storage granules) is found normally in the liver, spleen, and bone marrow.

What Do Pathological Values Mean?
 
  • Increase: Burns, presence of cold agglutinins, hemochromatosis, hemolytic anemia, haemosiderosis, megaloblastic anemia, paroxysmal nocturnal hemoglobinuria, pernicious anemia, renal tubular destruction, sickle cell anemia, thalassemia, multiple transfusions.
