Cyclosporine (Sandimmun) is an immunosuppressive agent derived from the fungus Tolypocladium inflatum. Cyclosporin is used to control graft rejection, especially in organs such as the liver, heart, and kidneys. The efficacy of cyclosporin is due to the specific and reversible inhibition of immunocompetent lymphocytes in the G0 and G1 phases of their cell cycle. T-cells and especially helper T-cells are selectively inhibited, although suppressive T-cells may also be suppressed. Cyclosporin also inhibits the production and release of cytokines, including interleukin 2.

Cyclosporine has many interactions with other drugs, resulting in an increase in neuro- and nephrotoxicity when co-administered with antibiotics, antifungals, or other immunosuppressants.

Cyclosporine has a narrow therapeutic range, with frequent side effects making it necessary to monitor its blood concentration.
