Α2-macroglobulin is one of the largest plasma proteins in size. It transports hormones and enzymes, has regulatory and inhibitory functions during the development of the lymphatic system, and inhibits components of the complement system and hemostatic mechanisms.
Elevated levels of alpha 2-macroglobulin are found in nephrotic syndrome when other low molecular weight proteins are lost, and A2-macroglobulin remains because of its large size. In patients with cirrhosis and diabetes, A2-macroglobulin levels are also elevated.
Patients with acute pancreatitis show low levels of A2-macroglobulin in the serum, and the extent of its decrease is related to the severity of the disease. Following serious surgeries, sepsis, and severe hepatic failure, A2-macroglobulin levels are often low. Patients with acute myocardial infarction and low A2-macroglobulin levels have a better prognosis.
